Your donation to the hospice care fund makes a significant impact on the lives of our patients. We are grateful for the outpouring of community support to help others spend those last precious moments surrounded by loved ones.

Compassionate Hospice Care

Our hospice professionals are specially trained to manage pain and symptoms with palliative medicine, so patients are relaxed and able to enjoy each day to the fullest extent possible. Avera@Home provides hospice care to more than 60 counties across South Dakota, Minnesota, Nebraska and Iowa.

Heartfelt generosity in support of hospice patients and their loved ones means others in your community have access to compassionate end-of-life care. Avera provides hospice care in many settings, long-term care facilities, assisted living centers and homes, including rural and remote areas.

Across dozens of communities in our region, the Avera hospice team provides in-home hospice care. In some Avera hospitals and our hospices houses, patients have tranquil settings for prayer, visitation and comfort. These locations provide home-like design plus medical technology to provide care for patients and support to their families.

Financial Relief for Families

Avera@Home hospice services are covered under Medicare and some Medicaid plans when services are provided in a residential home. Hospice care provided in a nursing home, independent or assisted living environment may include additional costs not covered under the benefit.

Many insurance policies and other managed care policies include hospice coverage for all or a portion of related costs. When patients don’t have insurance, Avera will work with them to identify ways to pay for hospice services.

Heartbeat Program

Losing a loved one is very difficult. Oftentimes, friends and family find solace in keepsakes to remember and cherish heartfelt moments.

The Heartbeat Program is offered to patients in hospice care at Avera. A board-certified music therapist records the sound of a patient’s heartbeat to be blended with a melody of their choice. Some choose to select a special tune or cover song. An individual can also write a song themselves with the assistance of a music therapist.
